Consent to Groom

I give Animal Care Center consent to groom my pet. I recognize that pets are unpredictable and sometimes accidents happen while being groomed. If there are any medical complications that occur during grooming, I understand that the veterinarian will stabilize my pet and then call me with an update. I also understand that if the groomer determines that my pet is matted to the point that a shave procedure is necessary and I have not signed a shave consent form that the groom will not be performed until verbal consent is obtained. If I am unreachable, I understand that my pet will not be groomed that day and will need to reschedule another appointment.
